Mandatory Requirements - All of these criteria must be completed. These criteria will be certified by the FJMC or by the Regional President.
1. On-line Registration for the 2012 World Wide Wrap. Yes / No
2. Purchasing and distributing FJMC Yom HaShoah Yellow Candles™ by May 31, 2012. Yes / No
3. Having dues paid in full to the FJMC as of May 31, 2012, and not having any past due balances. Yes / No
4. Having a membership list submitted between June 1, 2011 and May 30, 2012. Yes / No
5. Having a member attending a regional retreat, OR an FJMC consultant-led regional or individual club training program. (The January 2012 LDI and FJMC Keruv training will count for this criterion.). If none of these options is available, a visitation by an FJMC International or Regional Representative may be substituted. (This item to be certified by the region president)

- Have a club member attend the 2011 FJMC convention (other than congregational clergy) Yes / No
- Purchasing FJMC books or materials other than Yellow Candles Yes / No
- Having a member as an elected FJMC or Regional officer (i.e. not a member of a Board of Directors or Trustees) Yes / No
- Having a member (other than one in #3) actively serving on an International FJMC committee Yes / No
- Having a member attend a regional honoree program, such as a man-of-the-year event, OR some other regional event that is not training or a meeting. A meeting with a program is acceptable. Yes / No
- Holding a program to enhance Jewish observance (other than distributing yellow candles or holding a Wrap) (Examples: Hebrew Literacy; Art of Jewish Living course; Torah study with the Rabbi; selling sukkah kits; Build-A-Pair Program, etc..) This is meant to include some kind of hands-on activity and not simply a Sunday morning speaker on the matter. Yes / No
- Holding a community service project (Holding a community service project elsewhere during the year; examples: delivering food to the needy; holding religious services at an assisted living home; collecting books and toys for an orphanage, etc.) Yes / No
